This is the simplest DIY low pass filter for subwoofer using uA741 single op-amp ic. The circuit performs really well compared to it's price. The cut off frequency of this circuit is 25Hz to 80Hz maximum.
Using this circuit , you can easily design a 2.1 Subwoofer and Speaker System at your home.
The circuit is commerially available in market, contains very few components. You can easily make one by following the instructions in this tutorial.

I have seperately subwoofer circuit it require input. i want this circuit board is connect my 2.1 amplifier using la4440 ic .i dont know how to connect woofer circuit in amp.which point is input to woofer circuit in amp.

The smaller section is the power supply, you don't need to connect the power supply separately. And you can make the circuit even on a beadboard.

No. This circuit is made from an OPAMP. A signal from a PHONO, CD, or TAPE is too low in voltage (and signal strength), and cannot drive a WOOFER speaker. What you have to do is apply the PHONO, AUDIO CD, or TAPE as an input to this OP AMP circuit. Then, this opamp circuit can be connected to a PREAMP, and the PREAMP connects to a power AUDIO AMPLIFIER, and the AMPLIFIER drives the WOOFER or the SUBWOOFER. This circuit is just a filter. The opamp and components remove the high frequency parts of the audio signal, and leave only the boomy bass part. That bass part goes to the PREAMP, and to the AMPLIFIER to drive the WOOFER speaker.
REMEMBER, the opamp is a very low output swing, low output current device.
